DOS Student Conduct & Community Standards - Advisor Notification Form


THIS FORM MUST BE COMPLETED AT LEAST FIVE DAYS IN ADVANCE OF YOUR CONDUCT MEETING IF YOU PLAN TO HAVE AN ADVISOR PRESENT THAT IS AN ATTORNEY

The University of Illinois at Chicago (UIC) Student Disciplinary Policy stipulates that a student may have an advisor with them during the student conduct process. An advisor may be a family member, friend, faculty or staff member, advocate, or an attorney. One of the primary goals of the student conduct process is that of being an integral part of the student’s total learning experience. Students in our conduct process have the primary responsibility for presenting their perspective of an incident, with the advisor serving in a supportive and advisory capacity. The advisor may not represent or speak for the student. Although they may consult with their advisor during the hearing, this consultation must take place in a manner that does not disrupt the proceedings. Additionally, an advisor may not serve as a witness. A breach in any of these procedures will usually bring about a warning(s). After a warning, any additional breach will likely result in the Advisor being required to leave the hearing.

If an attorney will be your advisor, the Office of the Dean of Students must be notified through this form three (3) days prior to the conference or hearing. When an attorney is present as an advisor, the University may have an attorney from the Office of University Counsel in attendance as well.

The Office of the Dean of Students and/or the Office of University Counsel will not be able to directly communicate with your advisor without you completing the Consent to Release Records (FERPA). If you choose to provide consent to release your educational records, you must complete the online form.
