Instructions: Please complete the following form if you are interested in joining the Career-based Asian American Mentor Program (CAAMP) for the Fall of 2024. Response receipts and submission edits have been enabled on this form. To save your app, please select "No" on this form's final question before submitting, and check your email for the receipt to edit. To submit your completed app, select "Yes" before submitting. 

AAMP pairs students with Asian American and Pacific Islander undergraduate students with graduate students and working professionals to who help you navigate your journey to graduate/professional schools and beyond. The program introduces you to the Asian American communities at UIC and helps you professionally develop while making lifelong friends and connections.

For priority consideration, please complete this form as soon as possible and no later than April 22nd.

**Please note that this form will only be viewed by the Associate Director of the Asian American Resource and Cultural Center and AAMP Graduate Assistant and data is used purely for institutional reporting purposes and mentor/mentee pairing.**
