All students interested in filing an appeal in regards to sanctions issued please complete the following information listed below. Please note you will need your case number when submitting this request. Your case number can be found on your outcome letter that was delivered to you detailing your sanctions.
Instructions for the Appeal Form:
1. The Appeal Form and any supporting documents must be completed and typed by the individual student.
2. The reason(s) for the appeal must be selected on the Appeal Form.
3. All appeals for the student conduct process must be submitted electronically through this Appeal Form.
4. The appeal decision letter via your UIC email account.
5. The Appeal Form must be received in correspondence within five (5) business days after receipt of the written notification of a disciplinary outcome letter. Late appeals will NOT be accepted.
If sufficient grounds for appeal are evident, the Appeal Committee will take action on the appeal. The Appeal Committee may reduce the severity of the sanction, increase the severity of the sanction, or sustain the sanction. When circumstances warrant, as determined by the Appeal Committee the case may be referred to a different Hearing Board for additional proceedings.
If sufficient grounds for appeal are not evident, the Appeal Authority will deny the appeal and the original sanction will stand. The decision of the Appeal Authority is final.
Current Sanctions
The sanction(s) imposed as a result of the original hearing will not go into effect until such time an appeal decision is rendered and FINAL. Students can operate under their current schedule until an appeal decision is made. If students were issued interim interventions prior to the conduct hearing, those interventions remain active until the end of the student conduct process (e.g., the appeal decision).
