DOS Student Conduct & Community Standards - Sanction Submission Form


This form is used by students or student organizations to submit sanctions imposed for violating the University of Illinois at Chicago Student Disciplinary Policy or other associated policies. Please refer to the sanction letter for specifics about the sanctions you were assigned. Please write your name and UIN # on all documents being submitted. Also, be sure to sign and date all documents and forms when required.

Failure to properly complete forms will result in a delay in processing. After submission, please allow up to three (3) business days for your documentation to be updated in the Office of the Dean of Students Conduct Database as being received.
