The Office of the Dean of Students - Student Emergency Absence Notification Form


Please note that a student's individual request can take up to 48 hours for processing.

The Office of the Dean of Students – Student Assistance can provide action regarding requests for verifying absences due to documented circumstances, including medical emergencies or personal/family emergencies. Submitting this form will initiate the absence notification process. If you have specific questions regarding absence notice, please contact us at 312-996-4857 during regular business hours (Monday through Friday, 8:30am to 5:00pm). If you have an emergency or a matter requiring immediate attention, please contact UIC Police at 312-355-5555.

Final Approval of all absences, missed work, and/or extended academic deadlines is determined by the instructor.

Your Absence Notification request WILL NOT be processed without documentation. You can attach documentation to this request form. DO NOT
 upload confidential or sensitive material such as hospital records, credit card statements, etc.  You may also email or hand deliver to the Office of the Dean of Students for formal notification.

This form is not intended for COVID-19 reporting and/or verifying absences related to COVID-19. Our office does not oversee academic decisions and therefore cannot make accommodations for virtual learning, nor can we excuse absences or provide extensions on assignments/exams. Similarly, the DRC is not providing virtual learning accommodations for students who test positive for COVID or need to quarantine because of COVID exposure.

The official UIC guidance states that students who test positive for COVID -19, were in contact with an individual who tested positive for COVID-19, OR are exhibiting symptoms of COVID-19, should work with UIC Contact Tracing to make a formal report. Contact Tracing at UIC can be contacted at uic_contact_tracing@uic.edu or by phone at (312)355-2555. Contact Tracing will provide additional guidance related to your quarantine requirements If you have already made this report, and/or tested positive at a UIC saliva testing facility, please wait to hear back from Contact Tracing and contact your faculty members regarding your situation in the meantime.

This form should be completed by current students; however, submissions not submitted by current students will be reviewed for consideration based on extenuating circumstances (i.e., a family member or other making this request on behalf of the student). Please note that unexpected circumstances do occur and can cause a delay in review, but every effort is made to process all requests as efficiently as possible.

Our office may take steps to call medical providers or others to validate any documentation submitted. Submission of falsified documentation by a student may result in charges under the Student Code of Conduct and inability to utilize absence verification in the future.(Required)

Your Absence Verification request WILL NOT be processed without original documentation provided to the Office of the Dean of Students – Student Assistance. You can attach supporting documentation directly to your request form by uploading them below. PDF format is preferred.
